Please note that a Superbill will be generated at the end of each month. If you are hoping to submit this to insurance for possible reimbursement, please let Therapist know during the intake session so that the document will reflect the needs of the insurer.
The following session lengths are available for scheduling:
Initial Intake Session: 53-60 minutes
Standard Individual Session: 53-60 minutes
Play Therapy Session: 45-53 minutes
Couple or Family Session: 53-60 minutes
Private pay cost per session:
Initial Intake session: $150
Individual Standard sessions: $150
Play Therapy sessions: $150
Couple or Family sessions: $200
Evaluations for Emotional Support Animals (ESA):
50-minute Clinical Assessment: $150
ESA Letter: included in this fee if Therapist determines that an ESA is clinically appropriate. Assessment fee applies regardless of outcome.
OTHER FEES:
Cancellation Fee: $150
We reserve your appointment time exclusively for you. We require at least 24-hour cancellation notice so that the time can be offered to another client in need. If you cancel late or fail to show for your appointment without appropriate notice, you will be charged a $150 late-cancellation/no-show fee.
NSF/Charge Back Fee: $35
If your payment check is returned as Non-Sufficient Funds (NSF), or your credit card company reverses your payment, you will be charged a $35 fee to cover our costs.
Medical Record Requests: $50
This fee is charged for compiling and sending your medical records to a third-party.
Documentation Fee: $150 per hour, billed in 1-hour increments
This fee is to compensate for the time invested in completing forms on your behalf (e.g. FMLA, Disabiity, Hospitals, Insurance, etc.).
Court Testimony or Appearance Fees:
In the event that Therapist is voluntarily or by court order/subpoena to participate in any legal proceedings, a $2,000 minimum fee will be charged. 72 hours before the scheduled court date, $1,500 will be billed to cover required services, which are billed at $250 per hour. The remainder of the costs will be billed directly after the court appearance and will be due upon receipt. Further break down of the fees are outlined in intake documentation.
Please note that Allied Therapy Center DOES NOT perform court evaluations.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

Be assured that there is no surprise billing. We do not offer any additional services beyond what is listed in the "Rates" section above (with the exception of our documentation fees and court-appearance fees which are outlined in our intake paperwork) and therefore you will be not be billed for anything beyond the fees as described. If any new billable services are offered beyond what has been described, you will be given an estimate for your approval before that service is provided.
